PRISON CHAPLAINS

Please
complete this form and send it to us even if your facility issues a
purchase order. We need all of the information on this
form to process your order.

Along
with this order form we need a letter from a person in authority on
your institution letterhead stating that you will accept the
baptistry as a gift from ARM Prison Outreach and will be
responsible for shipping charges.

The
baptistry must be shipped to and placed in the correctional facility.

The
baptistry will be shipped prepaid. We will send you an invoice to
be reimbursed within 30 days of the invoice date.
MILITARY
CHAPLAINS

Please complete this form, photocopy it and send the
copy directly to us. Even if a purchase order will be sent, we need
a copy of this form to process your order.

The military unit is required to purchase the baptistry and pay all shipping charges.
Overseas orders must arrange for transport outside the U.S.

MISSIONS

American Rehabilitation Ministries will pay shipping
costs to any point in the continental U.S. Overseas orders must
